Appointments

                Appointments can be made in advance.  However, if you or someone in your family has an accident or suddenly becomes ill, please call us and we will arrange to see you as soon as possible, preferably on the same day.

We want your personal health needs to receive the attention they deserve.  Depending on the issues at hand, we may devote multiple sessions to explore them in detail.

We respect your valuable time.  However, emergencies sometimes occur and can make the doctor run behind schedule.  In the event of lengthy delays or cancellation of an appointment due to emergencies beyond the doctor’s control, we will try to notify you so you can rearrange your schedule.

                To cancel or reschedule an appointment, please call 24 hours in advance so that someone else can be seen.

Bringing all Meds / Supplements You Take (or an Updated, Complete List of them) to all Visits

To help us practice the best holistic primary care, please bring all prescribed medicine, over-the-counter pills and remedies, as well as any supplements to your appointment.  A Complete list of all the above will also do.

 

Lab / Test Results

                Unless you have an appointment to follow up on the results, we will notify you of them by mail or by phone.  If you do not hear from our office regarding any test results three weeks after you have done the tests, please give us a call to confirm that we have received them. 

                Tests ordered by other doctors and specialists generally have to be reviewed by them.  

 

Payment & Fees

Unless you make arrangements ahead of time, payment and co-pays are due at the time of your visit.   We accept personal checks, cash, credit cards (Visa & Mastercard), and most insurance plans.  Please bring your current insurance card to each visit.

 

Records

                All records are confidential.  We will not discuss your health condition even with your family/close friends unless you give us permission.  

We will be happy to provide copies of your records with your signed requests.  Payment to cover the material and staff cost for copying is due before delivery of the copies.

 

Prescriptions

                Prescriptions are renewed during office hours.  Kindly call or fax us at least a week before you run out of your regular medicine.  Please have the pharmacy phone number, the names and dosages of the medications ready at the time of your request.

Antibiotics and all controlled substances including narcotics will not be prescribed without a visit, unless you have a prior contract after due examination & discussions.

 

After Hours

                We are available for our patients through an after-hours call system, 24 hours a day, 7 days a week.  You may reach us through our regular phone number for urgent matters.

 

Workman’s Comp / Disability / Liability Forms & Other Forms in which we have to Fill in more than Half a Page

                Because most of these forms ask very specific and detailed questions, we cannot reasonably fill them out without an interview and exam with the forms present.  Many forms also require you to fill out parts of them before doctor’s exam.  We ask that you fill out your portions and sign if needed before seeing us to avoid unnecessary delays for you.
